Two Marylebones, a few streets apart
The Marylebone most people picture is the Georgian grid around Marylebone High Street, with Harley Street and Wimpole Street to the east and red-brick mansion blocks filling the gaps. Many period blocks have a lift the size of a phone box, or none at all, and the bathrooms were fitted long before anyone thought about grab rails or a walk-in shower.
Walk north-west across Marylebone Road and Lisson Grove and the housing changes to post-war council estates such as Lisson Green and the blocks around Church Street market. Here the lifts are usually bigger, but when one is out of service an upper-floor resident can be stuck. That matters on the day a hospital bed or a hoist is delivered.
For care planning, the questions are the same on both sides of Marylebone Road: can equipment get in, is there room to work safely in the bathroom, and can the front door be reached by a carer with a key safe or a porter. We go through all three at the assessment.
Getting a carer to the door
Marylebone is well served by rail. Baker Street has the Bakerloo, Jubilee, Metropolitan, Circle and Hammersmith & City lines, Marylebone station has the Bakerloo line and Chiltern trains, and Bond Street has the Elizabeth line. From our Lewisham office a carer can take a Southeastern train to Charing Cross and then the Bakerloo line straight up to Baker Street or Marylebone.
Buses run along Baker Street, Gloucester Place and Marylebone Road (routes 2, 13, 18, 27, 74, 113, 139, 189, 205 and 453 between them), while the 139 and 189 serve Lisson Grove and Church Street. Marylebone High Street itself has no bus route, which is worth knowing if someone is planning a supported walk to the shops.
Driving is the weak link. Paid bays in Marylebone that usually have a four-hour limit and a surcharge on older diesels, and everything south of Marylebone Road, including the High Street, is inside the Congestion Charge zone. Our visiting carers travel by public transport.

How do I arrange home care in Marylebone through Westminster Council?
Marylebone is in the City of Westminster, whose adult social care is run jointly with Kensington and Chelsea. You can ask for a needs assessment on Westminster's online referral form, or phone Adult Social Care on 020 7641 2500. A relative can make the referral with the person's agreement. A financial assessment then decides what, if anything, the council pays. If the council funds care, a direct payment lets you choose your own agency. Hospital discharge in Marylebone can come from more than one direction. The western side tends to come home from St Mary's in Paddington, while the eastern edge near Great Portland Street is closer to University College Hospital on Euston Road. Either way, call us while the person is still on the ward so the assessment can happen around the discharge date.
When visits are not enough
Live-in care. A live-in carer needs a bedroom of their own and proper breaks. In Marylebone that rules out many one-bedroom mansion flats, but larger flats and the houses on the quieter streets often have the space.
