Yes. Let us know the moving date and the new address, and we'll reassess the new flat for access, the bathroom and any equipment, so the care plan matches the new home.

What happens if the lift is out of order on a visit day?

Carers will use the stairs to reach the flat. The bigger risk is someone being stuck inside, so we agree a plan at the assessment for appointments and emergencies if the lift fails, and we report repeated faults to the family.

Which hospital will Walworth residents be discharged from?
