Can a carer visit every day in Chiswick?

Often, but not always, and we will say which before you plan a week around it. The journey from Lewisham High Street to Turnham Green or Gunnersbury is roughly an hour to 70 minutes by Southeastern and then the District line or South Western Railway. That makes a longer morning visit for washing and dressing, or a sit-in so a husband or daughter can get out for the afternoon, much easier to staff than four short calls spread through the day. When the need really is round the clock, a live-in carer is the steadier answer.

Conversions, mansion blocks and riverside houses

Chiswick's homes shape the care plan more than most. Much of the area is Victorian and Edwardian terraces and big semis, and many of them were split into flats long ago. A first-floor conversion off Chiswick High Road usually means a steep shared staircase, a narrow landing and a bathroom with a bath, which is where washing safely becomes the problem. Hoists and stairlifts are often awkward to fit in those stairwells, so the plan may move the bed downstairs or switch to a strip wash with support.

The red-brick houses on the Hounslow side of Bedford Park, and the Georgian terraces of Strand-on-the-Green and Chiswick Mall, are bigger homes. They still spread over several floors, and the riverside lanes bring their own access questions. In the mansion blocks and council-built blocks nearer the A4, the first question is whether there is a lift and how reliable it is, because that matters as much as the carer when someone can no longer manage stairs.

Parking in a W4 controlled zone. Much of Chiswick is in controlled parking zones. If you get regular carer visits, Hounslow Council issues a free carer parking permit for a year, with a doctor's statement. It does not cover pay-and-display-only bays, disabled bays or council car parks.

Discharge from Charing Cross as well as West Middlesex

Chiswick sits at the eastern edge of Hounslow, and many residents are treated at Charing Cross Hospital on Fulham Palace Road, run by Imperial College Healthcare NHS Trust, as well as at West Middlesex University Hospital in Isleworth. That matters when someone is coming home: the ward team plans the discharge, but the council that assesses ongoing care is still Hounslow, because that is where the person lives. Stations, buses and the health centre

Turnham Green (District, with Piccadilly trains at quieter times), Stamford Brook (District) and Gunnersbury (District and Overground) serve the northern half; Chiswick and Kew Bridge stations on the South Western Railway Hounslow Loop serve the south. Buses along Chiswick High Road run towards Hammersmith and Brentford. Several GP practices are based at Chiswick Health Centre on Fishers Lane.

W4 is in the London Borough of Hounslow, so a needs assessment starts with the council's Adult Social Care First Contact Team: use the request support page or ring 020 8583 3100. Live-in care in W4

A live-in carer needs their own bedroom. In a Bedford Park house that is rarely a problem; in a one-bedroom conversion it rules live-in care out, and visits are the route. For a couple, one carer can support both.

Mum's flat is on the first floor of a converted house off the High Road. Can she stay there?

Often, yes. We look at the stairs, the bathroom and where she sleeps at the assessment, and whether rails, a perching stool or a council occupational therapist visit would help. If she can no longer manage the stairs at all, we will be straight about that too.

Dad was admitted to Charing Cross, not West Middlesex. Does that change anything?

Only who plans the discharge. Imperial's ward team arranges it, Hounslow Council still assesses his care at home, and we can start once the plan is agreed.

Bedford Park is partly in Ealing. Which council do we contact?

It depends on the address. Bedford Park is split between Ealing and Hounslow, with the larger conservation area on the Ealing side, so a W4 postcode alone doesn't settle it. Your council tax bill shows which council to contact; we cover both sides.

Can a carer walk with my mother to Chiswick House gardens or along the river?

Yes. Walks to Chiswick House and Gardens or along Strand-on-the-Green can be written into the care plan as companionship time.
