Home Care in Sands End, Fulham

Carers for terraced houses off Wandsworth Bridge Road, the council estates around Townmead Road and the riverside blocks at Imperial Wharf and Chelsea Creek.

We can arrange home care in Sands End, both visits and live-in care. Of all the parts of Hammersmith and Fulham, this is one of the easier ones for carers to reach from south-east London, because Imperial Wharf station is on the Overground from Clapham Junction. We start with a free home assessment by Jackie, our registered manager, then write a care plan with you. Visits still depend on having a carer free at the times you need, so tell us the times when you call.

Who pays for home care in Sands End?

Start with the council. Hammersmith & Fulham doesn't charge for the home care it arranges, so if an assessment finds your mother or father eligible, those hours cost nothing. The freephone number for Adult Social Care is 0800 145 6095, or send the council's online contact form. A GP or community nurse can make the referral too.

Our care is private and paid for, and we give you a written price after the assessment, never a guess on the phone. An agency can help with what the council can't give: someone waiting for an assessment, care that needs more time than the plan allows, or round-the-clock live-in care. The borough page covers direct payments if you want to choose your own provider with council funding.

Three kinds of home, three sets of questions

Sands End was gasworks, wharves and industry for most of the last century, and the housing still shows where each piece of land went. Our assessment asks different things depending on which kind of home someone lives in.

  • Victorian terraces on streets like Hamble Street and Pearscroft Road. Narrow halls and steep stairs, and often a bath with no shower. Bathing is usually the first thing that gets hard, and an occupational therapist's visit for a bath board or a level shower is worth asking for at the council assessment.
  • Council estates of low and mid-rise blocks. Check whether the block has a lift and whether it works reliably, because a walk-up is very different for someone using a frame. A key safe by the flat door saves a lot of trouble at the entry phone.
  • Riverside apartments at Imperial Wharf, Chelsea Harbour and Chelsea Creek. Lifts and level floors make hoists and wheelchairs easier. The concierge needs a list of the carers' names, and a two-bedroom flat can take a live-in carer where a one-bedroom flat can't.

Can a carer drive into Sands End?

Yes, but only with a booking. Sands End sits inside the South Fulham Clean Air Neighbourhood, where cameras on residential streets east and west of Wandsworth Bridge Road fine through-traffic. Both halves of the scheme are now permanent. Since September 2025 there has also been a trial camera on Settrington Road.

The fix is the resident visitor permit. You, or whoever manages things for you, book the carer's car in on RingGo before the visit. That lets them through the cameras and also pays for parking in your controlled zone. Any H&F resident can do this for as many visitors a day as they need. If nobody is booked in, the carer gets the fine.

By train and bus. Imperial Wharf is on the Overground between Clapham Junction and Shepherd's Bush, and Fulham Broadway on the District line is a walk or a bus ride away. The C3 bus serves Imperial Wharf station, and the 28 and 295 run along Wandsworth Bridge Road on the western side.

Coming home from Chelsea and Westminster

Sands End's nearest A&E is Chelsea and Westminster Hospital on Fulham Road, just over the Kensington and Chelsea border, so that's where most discharges come from. Find out from the ward who is planning the discharge, and phone us once a date is pencilled in. If you need an extra pair of hands in the first weeks back, our hospital discharge care page explains what we do. After a fall, Bishop Creighton House's Keep Active volunteers can help someone aged 60 or over rebuild strength over up to nine weekly visits once community rehab ends. Referral comes through the NHS community rehabilitation team (020 7385 9689 for questions).

Respite and live-in care

Family carers need time off too. Short-term live-in cover lets them go away while the person they look after stays in familiar surroundings. A carer's assessment from the council (0800 145 6095) or from Carers Network (020 8960 3033) may help fund it. Read about respite care.

Sands End FAQs

Asked by Sands End families

Questions families ask us about Sands End. If yours is not here, ask us directly.

Ask a question
Dad lives in a one-bedroom flat at Imperial Wharf. Is live-in care possible?

Not with only one bedroom, because a live-in carer needs a room of their own. Regular visits are the usual answer. The building's lifts and level access make visiting care straightforward once the concierge knows who is coming.

I don't live in the borough. Can I book the carer through the cameras for my mother?

The visitor permit belongs to her as the resident, but you can manage it for her on RingGo with her details. Set it up before care starts so the first visit isn't held up.

Can a carer take Mum along the river?

Yes. Getting out is part of the care plan if she wants it, and the riverside paths at Imperial Wharf are flat. A companionship visit can be a walk and a coffee.

Is Sands End close enough for early-morning visits from Lewisham?

It's at the nearer end of the borough for us, because of the Overground to Imperial Wharf. Whether we can cover a particular morning time still depends on our carers' availability, so give us the times and we will answer honestly.

Does Fulham Broadway count as Sands End?

Not quite, but we cover both, along with Parsons Green and the rest of SW6. Tell us the postcode and we will tell you which hospital and transport details apply.
