Supported Living for Hammersmith and Fulham Residents

We don't have a supported living home in Hammersmith and Fulham. Our three homes are in Whitton, Kingston upon Thames and Uxbridge, and an H&F resident can move into one permanently or come for a respite stay once their H&F social worker agrees the placement. Whitton is the closest for most of the borough, about half an hour's drive from Hammersmith over Chiswick Bridge.

Which of our homes is nearest to Hammersmith and Fulham?

All three are west or south-west of the borough. Rough journey times:

  • Whitton from Hammersmith, Ravenscourt Park or Stamford Brook: 25 to 35 minutes by car along the A316 over Chiswick Bridge. By public transport, take the District line west to Richmond and then a South Western Railway train on to Whitton, about 40 to 50 minutes in all.
  • Uxbridge from Hammersmith or Barons Court: the Piccadilly line runs there without a change in about 45 to 55 minutes. From Shepherd's Bush or White City, take the Hammersmith & City line from Shepherd's Bush Market or Wood Lane down to Hammersmith first. Check the destination board, because some Piccadilly trains go to Heathrow.
  • Kingston upon Thames from Fulham: the District line's Wimbledon branch runs from Fulham Broadway, Parsons Green and Putney Bridge to Wimbledon, where trains continue to Kingston. Allow 45 to 60 minutes. By car, Putney Bridge is the crossing to use, since Hammersmith Bridge has been closed to motor traffic since 2019.

Who in H&F arranges supported living?

Supported living is agreed through Adult Social Care after a Care Act assessment. Which team you start with depends on where the person is now.

Nobody involved yet

Call Adult Social Care free on 0800 145 6095 (8.45am to 5pm, Monday to Friday) or use the online form on the council's contact us about assessments and care page. A GP, community nurse or mental health nurse can also make the referral.

Adults with a learning disability

The council's Learning Disability Service, on the 4th floor at 145-155 King Street, assesses adults aged 18 and over. The council says it helps with choices about housing, care and short breaks, so if your relative has a worker there, ask them about a placement outside the borough. The number is 020 8753 1900.

Young people leaving children's services

H&F's Integrated Adult Social Care Transitions Team (020 8753 2080, youngpeopleintransition@lbhf.gov.uk) works with young people moving from children's to adult services, including those with an active EHC plan, and carries out Care Act assessments. If your child already has a children's social worker or EHC coordinator, the council says to talk to them first. The council's preparing for adulthood page on independent living says the team also writes personalised housing plans with families and helps with tenancy skills.

Mental health needs

West London NHS Trust runs the borough's mental health services; the council lists its assessment team on 020 7386 1111 or 1113. If the person has a care coordinator, start there.

Turning 18 changes who can sign the tenancy. The council points out that if a young adult needs someone else to make decisions for them, including signing a tenancy or opening a bank account, that has to be formally agreed through the Court of Protection. Start early, because it can hold up a move.

How much does supported living cost for a Hammersmith and Fulham resident?

Rent and support are paid for separately. Rent is often met by Housing Benefit. Support is assessed by the council, and anyone with eligible long-term needs is likely to get a personal budget, says the council's assessments and paying for care page.

H&F is known for not charging for home care, a policy it has run since 2015. Don't assume that automatically covers support in a supported living placement, especially one outside the borough. Ask the social worker in writing what the person will be asked to contribute, if anything, before agreeing a move. People whose needs are mainly health needs may qualify for NHS Continuing Healthcare. We give a quote only after meeting the person and assessing the support they need.

Moving to Whitton or Uxbridge doesn't mean leaving H&F's care

When H&F arranges supported living in another borough, H&F normally remains responsible for the person's care and their reviews. Chapter 19 of the government's Care and Support Statutory Guidance sets out these ordinary residence rules. The GP usually changes after a permanent move.

Ties to home matter too. Someone who goes to the Nubian Life day centre in Shepherd's Bush, the Age UK activity centre on Greyhound Road or the Polish centre POSK on King Street can keep going. Put it in the support plan and agree how they'll travel back.

Two agreements: the tenancy and the support

In supported living the person holds a tenancy or licence for their room. Their support is a separate plan, delivered by a provider registered with the CQC for personal care. JNP Sidona is CQC-registered for personal care. We haven't been inspected yet, so we have no rating. What each home offers is on our supported living page.

What to ask any provider, including us

  • Who is the landlord, and what will the person sign: a tenancy or a licence?
  • Is there a ground-floor bedroom and a level-access shower if one is needed?
  • How is the home staffed at night, and who answers if something goes wrong?
  • Has the H&F social worker approved the placement, and when is the first review?
  • How will staff help with trips back to family in Hammersmith or Fulham?

Our son is in the Transitions Team's caseload. When should we mention supported living?

Now, if moving out at 18 or 19 is a real possibility. The council says the Transitions Team writes personalised housing plans with families, so raise it early enough for that plan to include a move out of the borough and visits to the home first.

We live in Fulham and don't drive. Is Kingston or Whitton easier for visits?

From Fulham Broadway or Parsons Green, Kingston is usually simpler: District line to Wimbledon, then one train. From the Hammersmith end, Whitton via Richmond is quicker.

Can someone come straight from Charing Cross or Hammersmith Hospital?

Only if the hospital and council teams agree it's the right next step and a room is free. Supported living is a home, not a place to recover in for a few weeks. If the person mainly needs short-term help, our respite care or hospital discharge support may fit better.

Can we try a short stay first?

Yes, when a room is free. All three homes take respite stays. If you're the family carer, ask Adult Social Care for a carer's assessment as well; Carers Network on 020 8960 3033 supports unpaid carers in H&F.
