- The Home and Settle Service. Run by Age UK East London with the NHS, it is free for up to four weeks for people with low to moderate needs whose discharge is held up by things at home: heating, shopping, a meal, collecting prescriptions.
- Newham Reablement. Free, short-term support for up to six weeks, usually three to four, to help someone relearn washing, dressing and cooking.
- The Discharge and Assessment Team. Newham Council's team that assesses people whose needs changed in hospital, once they are home.

Homes in Plaistow, and what they mean for care

Council services in E13
Several of Newham's own adult services are in Plaistow, which helps when care at home is combined with something outside it:
- The Resource Centre, 200 Chargeable Lane, runs the council's day opportunities for older people, a dementia service with Cognitive Stimulation Therapy and a rehabilitation gym. Referrals go through the Community Opportunities Team.
- The Direct Payments Team holds a drop-in at the Resource Centre on the first Friday of each month, 10am to 12 noon.
- Newham Memory Services, for dementia assessment and diagnosis, is at First Avenue Resource Centre, 103 First Avenue. Referral is usually through the GP.
Getting around Plaistow
Plaistow station is on the District and Hammersmith & City lines. Buses 69, 241, 262 and 473 stop at the station, and the 262 and 473 continue to Newham University Hospital. The Plaistow North and Plaistow South parking zones both run Monday to Saturday, 8am to 6.30pm.
